Your next step to complete registration:

It's a relief to know the policies, procedures, and permissions before investing in a program, so submitting Intake Forms is done before purchase.

STEP #1: Submit the 2022 Personal Success Dynamics Coaching Intake Forms by clicking the "GO TO FORMS" button below.  You will be taken to a new tab.  Once you have completed your submission, return to this page for Step #2.  If you want to preview the forms before clicking the start button below, you can view the forms here.

STEP #2: Once you have submitted the completed forms, click the "PURCHASE" button below if you are the Party Responsible for Payment.

Note:  When the adult client and the party responsible for payment are not the same person.  All Clients 18 years or older must submit forms even if they are not the party responsible for payment (ex., a parent paying for their adult child's membership).  If you are not the purchaser, you do not need to proceed to purchase after completing the Intake Forms.  However, the person purchasing the program for someone else must submit intake forms as the "Party Responsible" before they purchase.  Please take a look at the Intake Form for detailed instructions.

Note:  If you are registering more than one person (such as for yourself and your child) for the Personal Success Dynamics Coaching Membership, you will need to submit these forms more than once.  Please take a look at the Intake Form for detailed instructions.  Also, the system recognizes when you are already a member and will not let you make a duplicate purchase (i.e., purchasing multiple ADULT PSDC memberships for yourself, your spouse, and an adult child).  Therefore, you will need to log out and use an "incognito" or private browser window to purchase more than one identical membership. 

Note:  When purchasing an Adult PSDC membership, insert the personal details (name, email, etc.) for whose membership this is on the checkout page.  The checkout page creates the personal account and profile.  Then, use purchase information after clicking the "PayPal" button.  

You must submit these documents now to complete your registration.  If the system does not receive all forms before purchase, it denies your access to the coaching membership portal.
